Milady tells us that she and her brother have been searching for a doctor to cure Milady, but they've run into Dr. Manette, who's insane, and they're worried that he'll be able to cure their sister, but Milady doesn't seem to care. She's just glad that she isn't responsible for what happened to her brother and sister.
